Last week you developed your health promotion program, and in Week 5, you will propose an evaluation plan for the program that you designed in Week 4, and how that evaluation relates back to your chosen theory. This evaluation plan should be a minimum of 750 words in length. Please be specific if you are using a process, impact, or outcome evaluation or a combination of the three. In addition, create a visual presentation of your evaluation logic model. It can be presented in the form of a flowchart, a visual organizer, or any other format that is appropriate to the content.
